One thing I noticed in the pcap is that the provider does not send a ptime parameter in it's SDP packet. Only a maxptime parameter. Is that the case for all calls to this provider?
And regarding the packet-loss, have you enabled a QoS policy on the WAN interface to make sure EF marked packets will have priority when being sent to the WAN?
